Consent check box needed on customised Newsletter sign up page

How can I add the consent button to a customised Newsletter sign up page? I'm trying to use a form so it submits an email request and adds the requests to a database. If I could replicate the consent button that's on the New Account page (at the bottom) that would be great, so when we send newsletters or other promotions out through the site, the system will check the consent flag. The newsletter form would have to trigger the updating of the consent flag on the customer's account.
